Waiver & Consent

Please read the following carefully before signing. By completing this form you confirm that you have read, understood, and agree to the following:

  • I understand that nail services (BIAB, Gel Polish, Gel X Extensions) involve the use of chemical products, and I have disclosed all known allergies and sensitivities above.
  • I acknowledge that results may vary depending on the condition of my natural nails, lifestyle, and adherence to aftercare advice.
  • I understand that picking, peeling, or biting enhancements may cause damage to my natural nails. Bare by Den accepts no liability for damage resulting from failure to follow aftercare instructions.
  • I acknowledge that in rare cases an allergic or adverse reaction to products may occur, and I agree to notify my technician immediately if I experience any discomfort during or after my service.
  • I agree to undergo a patch test prior to my service if requested by my technician, particularly where I have disclosed sensitivities or this is my first visit.
  • I acknowledge that Bare by Den reserves the right to refuse or modify a service if, in the professional judgement of the technician, proceeding could cause harm.
  • I agree to provide at least 24 hours' notice for cancellations or rescheduling. Late cancellations or no-shows may incur a fee as per the current cancellation policy.
  • I confirm that all health information provided on this form is accurate and complete to the best of my knowledge, and I agree to advise Bare by Den of any changes at future appointments.
